Jukebox offers a wide range of home safety modifications; ranging from plug-in lighting and rug tape, to bathroom grab bar installation all the way to custom stairlifts, ramps, and elevators. The occupational therapist will make recommendations based upon your needs, and the safety risks identified in your home.
All confirmed enrollees of the UCLA Medicare Advantage plan are eligible for a complementary home safety evaluation to help you understand how your home safety or accessibility could be improved by minor home modifications.
If you have an eligible diagnosis you are eligible for an additional SSBCI benefit which covers up to $750 of products and installation.
Eligible diagnosis for this SSBCI benefit include:
- Cancer
- End-stage renal disease (ESRD)
- Stroke
- Chronic heart failure
- Dementia
- Diabetes
Yes! Jukebox Health will match you with a licensed Occupational Therapist to visit your home to learn more about your concerns, and to recommend simple safety improvements. We have found that in person visits are the most effective at correctly identifying risks and answering any questions you might have.
After receiving your Home Safety Report outlining recommendations, you'll work closely with a member of the Jukebox Health Client Care team to select products and modifications that best suit your needs and preferences. They will guide you through the selection process and assist in determining which items can be purchased using the benefit allowance outlined in your policy.
If you choose to move forward with any products and installation, Jukebox Health will use their network of licensed, insured, and background checked installers to professionally install these home modifications. All installers and projects are managed by the Jukebox Health team, and we can be reached at any time to discuss the status of your individual project.
