1. Submit E-Mod Request to Your LTSS Team
Submit a service recommendation and required assessments to the LTSS team.
2. Our OT Visits Member to Evaluate Needs
Our OT conducts an in-person dwelling assessment to clinically determine member need.
3. We Produce a Clinically-Guided Project Scope
We create a full project scope for your approval, combining our clinical and technical expertise.
4. Once Approved, We Execute Entire Project
When the project is green-lit, we capture bids and handle the technical execution of the project.
