New York, NY—Jukebox Health, the premier national provider of clinically-led home modifications, proudly announces the launch of its new Value-Based Care program, the “Independence Track.” The program combines a clinical approach with an innovative shared savings model to support Managed Care Organizations seeking to invest in member independence and control the rising costs of personal care services in the home. 

The majority of older adults and individuals with disabilities want to maintain their independence and remain in the homes they love. However, the rapid growth of the aging population, unsustainable home care costs, and worsening workforce shortages have made delivering consistent and effective care at home difficult. With Jukebox Health’s “Independence Track,” health plans now have an alternative to effectively address these challenges and ensure members can maintain and extend independent aging at home. 

Designed to serve individuals across Medicaid LTSS and Medicare-Medicaid Plans (MMP), the program leverages Jukebox Health’s clinically-led solutions to identify members who show signs of decreased mobility and deploy cost-effective interventions to prevent increasing personal care costs. By partnering with Jukebox Health, eligible members have access to:  

  • A comprehensive home evaluation conducted by a licensed and trained Occupational Therapist who identifies risks, assesses social care needs, and makes recommendations that will enable independence and improve health outcomes
  • Tailored product recommendations giving them the option to pursue home modifications to increase independence and support their activities of daily living
  • Professional installation of modifications by Jukebox Health’s vetted network of home modification and environmental service providers
  • One-on-one training with an Occupational Therapist to ensure proper utilization of the modifications to maintain and increase independence at home
